ATHOL - Anastasia Jessie (Hildak) Kozak, 86, formerly of Upton, died Monday, September 5, 2005 in Quabbin Valley Healthcare in Athol where she had resided for the past year. Her husband of 52 years, Michael Kozak, died in 1995.
Mrs. Kozak was born in Woonsocket, RI, a daughter of the late Nicholas and Josephine (Miezaniec) Hildak. She grew up in New York City, where she was educated in city schools and had worked as a mirror beveler for Arrow Mirror Company for many years. She lived in Brooklyn, NY, and then in Milford. MA for several years before moving to Millhaus Apartments in Upton, where she lived for 22 years. She was a communicant of Holy Angels Parish in Upton and a member of the Friends of Upton Elders.
She leaves one son, Michael R. Kozak and his wife Claudia of Wendell.
In lieu of flowers, memorial donations may be made to Holy Angels Parish, c/o the Rectory, 2 Nelson Street, Upton 01568.
Burial in Sacred Heart Cemetery, Medway Road, Milford, MA.
